Navigate Heart Health is an educational membership and peer-support community for people who have experienced a heart attack, stent placement, or bypass surgery.

It helps members apply secondary-prevention recommendations in everyday life and stay engaged in the health behaviors that support long-term cardiovascular health.

The membership is designed to complement clinical care, not replace cardiac rehabilitation, medical treatment, or individualized guidance from a licensed healthcare provider.

Created By A Registered Dietitian and Cardiac Rehab Professional

Navigate Heart Health was created by Melanie McAuliffe, RDN, LD, CCRP, a Registered Dietitian Nutritionist and AACVPR Certified Cardiac Rehabilitation Professional with more than 14 years of hospital-based clinical experience.

Her work in cardiac rehabilitation has included supporting thousands of patients as they navigate the medical, practical, and emotional challenges that can follow a cardiac event.

This combination of nutrition expertise and cardiac rehabilitation experience allows Navigate Heart Health to address more than food alone. The membership reflects the full secondary-prevention picture, including behavior change, movement, stress, sleep, social support, and ongoing engagement with medical care.

Navigate Heart Health may be appropriate for adults who:

  • Have experienced a heart attack, stent placement, or CABG surgery related to coronary artery disease

  • Feel overwhelmed by conflicting nutrition and lifestyle information

  • Are participating in cardiac rehabilitation or transitioning out of a program

  • Would benefit from ongoing education and connection with others who understand life after a cardiac event

    The membership is not designed for urgent medical concerns, individualized diagnosis, medication management, or one-to-one medical nutrition therapy.
